How do I add my logo to my intake forms?
There are 3 ways to use your own branding on all your intake forms:
Method 1
- Make sure your logo is already in the desired size. If not, use an image editor to resize it.
- On the top menu, go to "More" -> "Settings".
- On the Intake Form Header section, make sure the "Enable Custom Header" option is checked.

Drag your logo from your computer and drop it in the text editor or click on
- Select the image and resize it by adjusting its borders with the mouse.
- To align the logo left, select the image, click on the Edit button and choose the option "Left" in the position field.
Method 2 (requires HTML knowledge)
- On the top menu, go to "More" -> "Settings".
- On the Intake Form Header section, make sure the "Enable Custom Header" option is checked.
- Place the cursor inside the text editor box.

Click on the "Toggle HTML" button in the toolbar:

Write the HTML code necessary to display the logo. Example:

Method 3
Send us your logo, along with a screenshot or a description of how you want your header to look like. One of our designers will create the HTML code for you.
