So how does it work?

1) The first step is to create your intake package, consisting of an intake form and as many consent/agreement forms as you need (consent to treatment, office policies, HIPAA release, etc). Alternatively, you can send us your existing forms so we'll create the package for you.

2) Once your forms are set up, you can send them privately to your patients or put them on your website.

3) As soon as a patient completes and submits an intake package, you receive a notification and the documents become available in your IntakeQ account.

4) You can then review it, add note to it and export to PDF. Many of our clients download the intake form and upload it to their EHR system, including the e-signed consent forms and agreements.
