If mom OR baby has BCBS or Anthem:
If either mom or baby has a BCBS or Anthem plan, please use this button first, even if the other person has different insurance.
If BOTH mom and baby have Cigna:
If both mom and baby have Cigna, please use the portal below to verify your eligibility and get started.
If either mom or baby has BCBS or Anthem, use the BCBS/Anthem button above instead. If either mom or baby has UHC or Aetna please scroll all the way down to the bottom of the page and fill out the form.
Scroll to the bottom of this page and complete the form below if:
- Both mom and baby have United Healthcare, UMR, Surest, another United Healthcare affiliate, or an Aetna plan.
- Mom and baby have a combination of Cigna, Aetna and/or United Healthcare (or another UHC affiliate).
- You are using Samaritan Ministries, Christian Healthcare Ministries (CHM), another Health Share plan, or choosing to self-pay.

Quick Note
Some plans, including Tricare, Medicaid, STAR, CHIP, HMO, and Marketplace plans, are not currently in-network and are typically self-pay. If you're unsure which option applies to you, don't worry—we'll help guide you.
