Screening Interest Form

The first step in getting started with us is a complimentary screening. The purpose of a screening is for us to learn more about the concerns you have with your child's development, for us to tell you about us, and for us to determine if we are the best place to help (and if not help you decide what is). Please complete this form and someone from our office will contact you within 48 hours to answer any preliminary questions you have. Because of the volume of screening requests you will be put on a waitlist until we have an opening for a screening call.
