We are excited you have decided to GET STARTED with SKILLS ON THE HILL!

What happens next?  

1. Please submit your information in the below HIPAA secure form.  Be as specific as possible and make sure to complete all fields including the insurance information and card images. 

2. Out Benefits Team will contact your insurance to get details about your plan and it's benefits for the services you indicate you are interested in.

3. Be on the look out for a form that arrives in your inbox from Skills on the Hill.  It will be titled "PLEASE FILL OUT THIS FORM" from office@skillsonthehill.com.  This form is a Financial Agreement that we are obligated to provide as part of the No Surprises Act.  In order to proceed with scheduling, this form must be reviewed and signed by the patient's guardian.

4. After the Financial Agreement is signed, we will then send you a link to complete our intake paperwork.  Once this is done, we will be able to schedule the patient for services!
