Momentum Lactation recognizes that there are sometimes financial circumstances that make it difficult to afford the cost of lactation care. Insurance coverage for lactation care often varies greatly between carriers requiring patients who need care to pay out-of-pocket. We do not want this to become a barrier to families seeking lactation or postpartum support. We offer a sliding fee scale based on household size and income level, and encourage families to complete a brief financial assessment to determine their eligibility for sliding scale billing.
POLICIES:
- A nominal fee of $25 will be collected at the time of service for all patients regardless of qualifying discount.
- Patients receiving a discount will pay both the $25 fee plus their discounted rate for services.
- Example: Suzie qualifies for a 70% discount based on her income and family size. She books an appointment for a prenatal visit ($100). Suzie will pay $25 (service fee) + $30 (fee for appointment with discount applied) for a total of $55.
- In order for patients to qualify for a discounted service rate, the online financial assessment must be completed prior to the scheduled appointment with Momentum Lactation. The visit will then be billed according to the qualifying discount level.
- Your sliding scale discount will remain active for a 12 month period following your application. After 12 months, you will be required to resubmit your application to determine whether your circumstances have changed and whether your subsidy level is still appropriate.
- After submitting your financial assessment form, you will be notified via email regarding your qualification for a discount.
