Please note that we are not currently able to accept new patients who are seeking ADHD treatment due to increased regulatory scrutiny surrounding its diagnosis and treatment.

We are also not able to conduct neuropsychological testing for ADHD, which must be completed by a neuropsychologist. 

If you would like referrals for ADHD testing and/or treatment, please call us at 330-858-1173.

Once you submit the form and it is reviewed, our office will send you the intake packet for prospective patients. Please check your email spam folder if you haven't received it within 3 business days. Once your intake packet has been submitted, our office will call you to schedule an appointment, or we will provide referrals in the event that we are not able to meet your treatment needs.
