Complete a brief initial form to help us confirm that our practice’s clinical expertise and administrative resources align with your specific goals.

2. Secure Your Consultation

Upon successful completion of the screen, you will be directed to our calendar to select a time for your initial visit and secure your appointment.

3. Finalize Your Intake

After booking, you will receive your comprehensive clinical intake paperwork via email. These forms must be completed within 48 hours to confirm your appointment. Dr. Lee will personally review your history prior to your arrival to ensure your first session is as meaningful and productive as possible.
