Confirm (cancel or reschedule) an appointment
Due to COVID-19 epidemic, all patients must have confirmed appointments so we can triage and accommodate patients safely and in timely manner. Due to high number of no shows, ALL SCHEDULLED APPOINTMENTS MUST BE CONFIRMED ONE DAY PRIOR TO APPOINTMENT’S DATE (not earlier), any unconfirmed appointment will be changed to Walk-in (patient will be seen only if schedule allows). To avoid cancellation, please use this form to confirm your scheduled appointment one day before appointment's date.
