Care Beyond the Classroom Secure Access

Thank you for your interest in Care Beyond the Classroom. Because this service may involve minors and confidential disability-related, educational, health, or personal information, access is carefully controlled.

To begin, the parent, legal guardian, or authorized adult representative should enter their own name and email address. A secure code will be sent to the email address provided. The code must be entered before the protected intake form can be accessed.

Please note:

• A minor should not complete this access request independently.
• The secure code verifies access to the email address provided; it does not constitute program approval or legal clearance.
• Additional consent, identity verification, authorization, documentation, or clearance may be required before Key Health Care can proceed.
• School, healthcare, or community representatives may initiate a referral only within the scope of their authority.
• Parent or legal-guardian permission may still be required.
• Please do not submit an IEP, 504 Plan, medical record, diagnosis, Social Security number, or other sensitive documentation unless specifically requested through an approved secure process.
• Do not share the secure access code with anyone.
