Upon form completion, you will automatically be sent to the booking page which will allow you to place an appointment spot on hold while your information is being reviewed. 

Placing an appointment on hold does not guarantee an appointment. Your information will be reviewed and the appointment will be confirmed or declined after review based on the clinic's current skill sets, availability, and financial policies. You will get an e-mail notice.
