This practice is only in-network with Horizon BCBSNJ Commercial Plans, PPO plans and out-of-state PPO Plans (Bluecard, Empire, Independence, ETC) BCBS. If your BCBS plan is an out of state based PPO plan, there will be a Suitcase symbol on your insurance card with a PPO inside.   

Also, Please confirm with BCBS if your MENTAL HEALTH COVERAGE is through BCBS. Some plans have mental health coverage through Beacon Health or other carrier for example.
