We work together to make sure you get connected with the right provider, in the right location, with the right coverage — without you having to figure it all out on your own.

If you were referred to a specific provider or found someone through their website or social media, you can select their name on the form when you fill it out (or write it in at the bottom), and we’ll make sure you’re connected with them if they serve your area.
If not, we’ll help match you with another amazing provider nearby.

If you have any of the following insurance plans:

  • Blue Cross Blue Shield (ANY state)
  • Anthem
  • United Healthcare / UMR
  • Cigna
  • Aetna

⚠️ Coverage note: Medicaid/STAR/CHIP, HMO, and Marketplace are not in-network and would be self-pay. Please check your insurance card before submitting. If you have one of these plans, we are not able to verify coverage or respond to submissions for these plans. You may continue to the form below and request a cash pay/self pay visit for these particular plans.
