Correctional & Justice Services Program Intake

*** After answering the following questions, signing the form, and making your payment, you may either call or email our Intake Worker with the Order # on your receipt to confirm payment for your program/s: please contact our intake worker by phone at 416-304-1974 x 209 or by email at deleone.downes@salvationarmy.ca. We can also receive text messages at 416-889-0913. Calls and emails are answered Monday to Friday between 9:00 am and 4:30 pm Eastern, excluding holidays. Our intake worker will have additional questions for you and will give you your program date and time.  Thank you.

IMPORTANT: YOU WILL NOT BE ENROLLED INTO THE PROGRAM UNTIL YOU HAVE CONFIRMED YOUR DATE, TIME, AND PAYMENT WITH THE INTAKE WORKER.
