Correctional & Justice Services Program Intake

*** After answering the following questions, signing the form, and making your payment, you may either call or email our Intake Worker with the Order # on your receipt to confirm payment for your program/s: please contact our intake worker by phone at 416-304-1974 x 209 or by email at deleone.downes@salvationarmy.ca. We can also receive text messages at 416-889-0913. Calls and emails are answered Monday to Friday between 9:00 am and 4:30 pm Eastern, excluding holidays. Our intake worker will have additional questions for you and will give you your program date and time.  You must submit your intake forms and the payment online first.Thank you.

IMPORTANT: YOU WILL NOT BE ENROLLED INTO THE PROGRAM UNTIL YOU HAVE CONFIRMED YOUR DATE, TIME, AND PAYMENT WITH THE INTAKE WORKER.
